Les,

There is no flat washer between the spacer and the disc brake bracket, just 
the top hat shaped spacer.  The only flat washer goes just under the axle 
nut as you have said.  The axle can be installed from either direction, it 
makes no difference, just a matter of personal preference.  I install mine 
from the chain side so that the nut is on the brake side.
